A day nursery carer is someone who looks after children that are too young to go to school or kindergarten for parents who must nevertheless go to work, perhaps because they are single-parent families, and so cannot take care of them in the daytime themselves. Often these day nurseries are run by people who themselves have small children, and so are well placed to offer such a service to other local people. Alternatively, a parent whose children are of school age or who have left home might find that providing this service is a reasonable way to do something satisfying and at the same time supplementing their income.
